Bruxism (teeth grinding) treatment with botulinum toxin injections

Bruxism is an involuntary habit of grinding or clenching the teeth, often linked to tension, stress and disturbances in sleep quality. In many cases, a person may not notice the habit until relatives point it out or a dentist observes characteristic changes associated with bruxism.

Over time, bruxism can thin the enamel, increase tooth sensitivity and lead to cracks. The jaw joints can become more sensitive and may produce clicking sounds. This condition may trigger headaches, facial or neck pain, and disrupt sleep.

One of the modern treatment methods for bruxism is botulinum toxin injections into the masticatory muscles.

At Primum Estetica, this procedure is performed to reduce jaw tension, slow down tooth wear and alleviate symptoms associated with bruxism.

How can botulinum toxin injections help in treating bruxism?

For bruxism treatment, botulinum toxin is injected into the masticatory muscles (most commonly the masseter muscle). By reducing the activity of the muscle:

  • involuntary clenching and grinding become less intense;
  • tension in the jaw and facial muscles decreases, including morning stiffness;
  • tooth surfaces wear more slowly, and restorations are better preserved;
  • sleep quality and overall well-being improve.

When are botulinum toxin injections not performed?

The procedure is usually not performed:

  • during pregnancy or breastfeeding;
  • in cases of allergy to botulinum toxin or its components;
  • when there is an infection or inflammation in the injection areas;
  • in the presence of certain neuromuscular conditions (e.g., myasthenia);
  • when blood clotting disorders are insufficiently controlled or certain medications are being used.

The doctor makes the final decision during the consultation after assessing overall health and the severity of bruxism.

How is the procedure performed?

A consultation is carried out first. During it, symptoms, sleep habits and previously tried treatments are discussed, and the condition of the jaw is assessed. An individual treatment plan is then created: injection sites are selected, the required amount of botulinum toxin is determined, and a possible repeat schedule is discussed.

During the procedure, the injection points in the masticatory muscles are marked. The injections are administered using a fine needle, and anaesthesia is usually not required. Only a slight prick and mild pressure may be felt.

How long does the procedure take?

Botulinum toxin injections take approximately 15–30 minutes. Including the consultation, the visit typically lasts up to 45 minutes.

What to expect after the procedure?

In the first few hours, the injection area may show:

  • mild tenderness or aching;
  • slight redness or bruising;
  • temporary “fullness” in the jaw.

It is recommended to:

  • avoid intense physical activity, hot baths and saunas on the same day;
  • avoid touching or massaging the injection sites for several hours;
  • follow the doctor’s individual instructions.

When does the effect appear and how long does it last?

  • initial improvements are usually noticed after 3–7 days.
  • maximum effect develops within 2–4 weeks;
  • results last around 3–6 months, depending on individual factors and lifestyle.

Is the procedure safe?

Botulinum toxin has been used in medicine for many years and is considered safe when administered by an experienced doctor in a medical setting.

As with any treatment, the effect may vary from person to person. For some patients it significantly reduces pain and grinding episodes, while in other cases the effect is moderate. The procedure may be combined with other recommended treatment options.

If you notice increased tooth wear, jaw tension or involuntary grinding, it is worth seeking a consultation. The doctor will assess your situation and offer the safest and most suitable treatment plan. At Primum Estetica, botulinum toxin injections for bruxism treatment are performed by dermatologist and venereologist Agnė Bartkevičiūtė.
